 Each exam takes about 15 minutes. $30 cash;  Must have completed the first page with physical history AND be signed by guardian prior to a doctor being able to see you. Download and bring the paperwork with you.  There will only be a few copies available. Easiest way to ensure that you have your physical exam completed for 24-25 athletics season.  Please come with your confirmation page from homecampus.com.  You do NOT need to upload your physical exam into home campus.
